Extension Agreement

An non-non-refudable investment is required at the time that your appointment is scheduled. Your stylist will quote the non-refundable investment price during your consultation. Any cancellation or changes to an appointment must be made 72 hours prior to the scheduled appointment, otherwise, your investment will be forfeited. You must then schedule a new appointment and make another investment. If you change the color or texture of your natural hair between the consultation and the installation appointment, you agree to notify Bella Chic Salon prior to the installation appointment in order to schedule a new consultation. A new investment may be required. Bella Chic Salon requires cash or credit card as payment. Bella Chic Salon will protect your privacy rights as a consumer and keep all information strictly confidential. Please keep the home maintenance sheet to ensure the best results. Bella Chic Salon will recommend the proper products and care routine to be used on the extensions. Bella Chic Salon and our stylists are not responsible for any damage caused to the extension due to improper care, whether you have an allergic reactiion to the beads, or if for any reason that may cause the client not to be able to wear the extensions. There will be NO REFUNDS issued for any reason on extensions. Loose hair cannot be distributed to any client that does not have a current cosmetology license. Hair will be stored only by Bella Chic Salon
